Investigators Found Illegal Aliens Getting Taxpayer-Funded Breast Implants and Hormone Therapy Inside San Francisco Homeless Shelters
City Journal reporters walked into San Francisco's largest homeless shelter – St. Vincent de Paul's MSC-South, running on a $66 million city contract – and found illegal aliens from Honduras, Mexico, El Salvador, and Venezuela receiving taxpayer-funded sex change procedures through Medi-Cal.
Medi-Cal is California’s state run Medicaid program.
A shelter employee showed the investigators around and confirmed that illegal aliens were living there.
Management, he said, instructs employees to refuse cooperation with federal immigration authorities.
One resident, a Honduran illegal alien going by "Lyca," confirmed everything when asked how he was receiving hormone therapy without documentation.
Two words: "Tengo Medi-Cal."
That's Gavin Newsom's California.
At the Embarcadero SAFE Navigation Center, another illegal alien from Mexico named "Jacqueline" walked investigators through the full pipeline.
"Even though you're undocumented, you can get them," he said – referring to breast implants paid for entirely by California taxpayers.
He confirmed he already had breast implants through Medi-Cal – and when investigators asked about genital surgery, his answer was three words: "Waiting for that one."
The investigators didn't stop there.
At the Taimon Booton Navigation Center – a shelter built specifically for men identifying as women and other trans-identified individuals – they found another group of illegal alien men outside the facility, all pursuing the same treatment pipeline.
Every shelter. Same story. Same program. Your money.

Investigators confirmed the pipeline is now well established in transgender communities across Mexico, Honduras, and beyond.
This didn't happen by accident.
Newsom deliberately expanded Medi-Cal to cover illegal aliens – classifying sex change procedures as medically necessary.
California has been on this path for years.
In 2017, Newsom's California became the first state in the country to fund sex change surgeries for prison inmates.
In 2019, Newsom expanded Medi-Cal to illegal aliens aged 19 to 25.
In 2024, he opened it to all illegal aliens regardless of age.
Every expansion, the same promise: California believes everyone deserves care regardless of immigration status.
Every expansion, the bill got bigger.
Newsom's Medi-Cal expansion came in $6.2 billion over his own projections in a single year.
California is now staring at a $12 billion budget deficit.
Newsom's fix is to cut GLP-1 diabetes medication through Medi-Cal starting January 2026 – along with dental benefits and drug coverage that legal California residents paid into for decades.
While he cuts their insulin, the illegal alien in that San Francisco shelter is still waiting on his surgery.
Neither the California Department of Health Care Services nor the shelters' executive directors responded to press inquiries.
They don't need to. Newsom signed it into law. The money flows automatically.
Trump has been called extreme for wanting to pull federal funding from states covering illegal aliens. This story is exactly why.
Newsom wants to run for president on a platform of California values. He's spent years telling the country that his state is the model – the proof that open borders and universal government healthcare can coexist without consequence.
The investigators who walked those shelter hallways just showed you the consequence.
Seniors losing their diabetes medication. Illegal aliens getting free surgeries. A $12 billion hole in the budget. And a governor who won't answer a single question about any of it.
Newsom built a system that rewards people for crossing the border and punishes the Californians who were already here. The investigators who walked those shelter hallways did not find a loophole. They found the policy working exactly as Newsom designed it.
He just never expected anyone to show it to you.
